We offer two main types of services through our online platform:

  1. Coaching Appointments: Focus on general well-being, providing guidance, recommendations, and signposting to relevant resources.
  2. Lifestyle Medicine Consultations: Targeted sessions to improve health through personalized lifestyle interventions.

Important Notes:

  • Coaching appointments are not medical consultations and do not provide medical advice, diagnosis, or treatment.
  • Lifestyle consultations are not designed to replace your NHS GP or private healthcare provider.

What We Don’t Do:

  • Provide emergency services or manage acute medical conditions.
  • Prescribe controlled drugs or certain specialist medications.

For urgent or life-threatening situations, always contact 999 or your local emergency services.

  1. Who Can Use Our Services

Our services are designed for adults who meet the following criteria:

  • Coaching Appointments: Available to individuals aged 18 and above, residing in the UK.
  • Lifestyle Medicine Consultations: Only available to individuals aged 18 and above living in England.

Unfortunately, we cannot provide services to:

  • Minors under 18 years old.
  • Individuals outside of England for Lifestyle Medicine Consultations.

The Lifestyle GP LTD  reserves the right to deny any person access to the Platform or the Services. We reserve the right to deny any person access for any inappropriate acts which may include but are not limited to:

  • inappropriate, abusive, offensive behaviour or remarks including on social media.
  1. Your Responsibilities

To provide you with the best possible care, we require accurate and truthful information, including:

  • Self-reported metrics like weight and blood pressure.
  • Medical history and any diagnoses or reports relevant to your health.

Why This Matters:
Inaccurate or incomplete information may affect the quality of advice or recommendations we can provide. We cannot be held liable for outcomes based on incorrect or misleading data.

We are committed to safeguarding your personal information.

What We Collect and Why:

  • To deliver our services, we may collect health information, personal details, and recordings of sessions for clinical governance.

How We Use Your Data:

  • Your information is confidential and will only be shared with your consent or if required by law (e.g., in emergencies, if you or others are at risk of harm or due to illegal activity.).

 

  1. Payment and Fees
  • Payment Method: Payments are made online via Stripe or BACS. We do not accept cash or other payment methods.
  • Fees: Appointment fees are listed on our website and are subject to change. However, any changes will not affect appointments already booked.
  • Refunds: 
    • Cancellations made more than 48 hours before the appointment are eligible for a full refund.
    • Late cancellations (within 48 hours) or missed appointments are non-refundable.
  1. Appointments
  • Booking: Appointments are made via our platform, and you’ll receive a confirmation email with session details.
  • Reminders: You’ll get a reminder email the day before your session.
  • Timeliness: Ensure you’re on time for your appointment. If you’re late, the session may be cancelled or shortened without a refund.
  1. Using Our Platform
  • Account Security: Keep your login credentials confidential. Contact us immediately if you suspect your account has been compromised.
  • Technical Requirements: Ensure your device and internet connection meet the necessary specifications.

Third-Party Links:
Our platform may link to third-party websites or services. We are not responsible for the content, accuracy, or privacy practices of these external providers.

  1. Cancellations and Rescheduling
  • Cancelations: Cancel up to 48 hours before your appointment for a full refund.
  • Rescheduling: You can reschedule up to 24 hours in advance without penalty.
  • Missed Appointments: No refunds for missed sessions or last-minute cancellations.
  1. Feedback and Complaints

We value your feedback and aim to resolve complaints quickly and fairly.

How to Submit a Complaint:

  • Contact us via email or post with the details of your concern.
  • Include your full name, date of birth, and appointment details.

What Happens Next:

  • We’ll acknowledge your complaint within 7 working days and aim to resolve it within 28 working days.
  1. Legal and Liability
  • Service Limitations: Our services are not suitable for all medical conditions and should not replace in-person care.
  • Our Liability: We are not responsible for issues caused by third-party providers, delays outside our control, or the misuse of our services.

Governing Law:
These terms are governed by the laws of England and Wales.
